Medical Assistance, CFSS, AC, ICLS, and private pay

Yes. Triple N is an enrolled Medical Assistance provider. MA is Minnesota's Medicaid program and covers home care for eligible individuals of all ages. Triple N bills MA directly — most clients pay $0 out of pocket.
Triple N does hold an Elderly Waiver license.
ICLS (Intensive Community-Based Long-Term Supports) provides support for adults with physical disabilities to remain living in the community. Triple N is an enrolled ICLS provider.
Yes. Triple N accepts private pay for clients who are not enrolled in a state-funded program or who prefer to pay out of pocket. Contact our office for current private pay rates.

Assessment, timeline, intake process, and first steps

Call (952) 898-4237 or submit a free assessment request online at triplenhomecare.com/contact. A team member will respond, confirm eligibility and funding, and build a personalized care plan with you.
For most clients, care begins within 1–2 weeks after intake and care planning. For urgent situations including hospital discharges, we work to start within 24–72 hours when possible.
A free assessment is a no-obligation meeting between Triple N and the client and family to understand care needs, confirm program eligibility, review funding options, and build an individualized care plan.
Yes. Triple N accepts referrals from hospitals, clinics, county social services, and managed care organizations. Submit at triplenhomecare.com/referral or call (952) 898-4237 directly.
