Individual Home Supports (IHS) is a Minnesota 245D licensed service providing individualized skill-building, daily living assistance, and community integration support for people with disabilities. Triple N Home Health Care is a licensed IHS provider serving children, teens, and adults in Burnsville and throughout the Twin Cities metro. What Are Individual Home Supports?
Individual Home Supports (IHS) is a 245D licensed service that goes beyond personal care — it focuses on building skills, increasing independence, and supporting meaningful participation in the community. IHS is delivered in the person’s home and in community settings based on their individualized support plan.
Unlike standard personal care, IHS is highly individualized. Every person’s goals are different — one person may be working on cooking skills, another on public transportation, another on managing their daily schedule. Triple N develops a support plan with the individual and their family that reflects those specific goals.
